The Healthy Families program (HFP) at PrimaryOne Health provides in-home services to any mothers and fathers from pregnancy until the child’s third birthday. The program is designed to ensure children are healthy, safe and ready to learn.

This program focuses on the following goals:

  • Increase healthy pregnancies
  • Improve child health, development and readiness
  • Improve parenting confidence and competence
  • Increase family connectedness to community and social support

This program is an affiliation with Healthy Families America and a part of the Ohio Department of Health Help Me Grow Home Visiting Program.

What is Healthy Families Program?

Healthy Families Program is for pregnant mothers and families. Families are paired with a Family Support Specialist who provide in-home, relationship-centered, and evidence-based care and support throughout pregnancy and continue to visit until your child is 3 years old. It is designed to promote healthy families and children through supportive environments and services that optimize child development, family goal plans, and empowered parenting.

How often will my family support specialist (FSS) visit?

This is a voluntary program and dependent on what you desire. Typically, your FSS will visit every week or two, during your pregnancy and continue with visits until your baby is 3 years old. You and your FSS will decide the exact schedule.

How much does the program cost?

Healthy Families Program is provided to patients at PrimaryOne Health at no cost to eligible women and families.

Can my baby’s father participate?

Healthy Families Program encourages fathers, family members and even friends to be involved in the visits and learn how they can best support you. You can have a conversation with your FSS to decide who you’d like to be involved.
