Q: Are we a free clinic?
A: No we are not a free clinic. We do offer a sliding fee scale for patients with limited income and assist patients seeking medical insurance.
Q: How is the fee I pay determined?
A: Sliding Fee is based on Proof of household income, and family size.
Q: Do I have to pay for my visit?
A: Even as a Sliding Fee patient there is a minimum patient responsibility. We do ask that you bring the minimum payment of $30 for the visit; however, this may increase based on income documentation. This does include the visit and any additional laboratory and in-house testing.
Q: Do I need a photo ID?
A: Yes, all patients are required to provide a current photo ID at each visit.
Q: Do I need to bring my prescription bottles and logs with me?
A: Yes, Please bring all medications you are currently taking (including over the counter medication), and dosage to your appointments.
Q: Are my medications free?
A: PrimaryOne Health patients have access to programs that may reduce the cost of eligible medications.
