I Have Insurance. What Do I Need to Bring to My Appointment?

We, at PrimaryOne Health, ask that you provide a valid photo ID, insurance card, social security number, all current medications (including over the counter medication), name and address of previous Primary Care Provider and proof of household income to your first appointment. All copays are expected at the time of service.

We, at PrimaryOne Health, ask that you provide a valid photo ID, social security number, all current medications (including over the counter medication), name and address of previous Primary Care Provider and proof of household income to your first appointment. All copays are expected at the time of service.

We offer a sliding fee discount for our uninsured patients based on a patient’s household income and family size. To apply for the sliding fee discount patients will need to provide proof of income for all members of the household (see below for acceptable income). Sliding fee charges will be determined once income documentation has been obtained and calculated.

If you need assistance in selecting an insurance plan through the Affordable Care Act Marketplace, PrimaryOne Health has Certified Application Counselors who are trained and able to help patients, as they look for health coverage options through the Marketplace, including helping them complete eligibility and enrollment forms. Their services are free to patients.

Ohio has granted permission for community health centers to participate in Presumptive Eligibility (PE) Program. The PE program allows us to provide immediate access to health services by giving our patients and their families, temporary health coverage through Medicaid if they are presumed to be eligible by us under the current Ohio Medicaid guidelines. To apply for temporary and ongoing Medicaid you will need to bring: 1 month of household income (current within the last 30 days), social security numbers for all family members requesting assistance, and identification for all family members requesting assistance.
