Need Help Navigating Insurance Options?

PrimaryOne Health is Here to Help!

If you need assistance in selecting an insurance plan through the Affordable Care Act Marketplace, PrimaryOne Health has Certified Application Counselors who are trained and able to help patients, as they look for health coverage options through the Marketplace, including helping them complete eligibility and enrollment forms. Their services are free to patients.

To schedule an appointment to speak with a Certified Application Counselor, visit your nearest PrimaryOne Health location or call 614-620-8088. If you do not reach our Outreach Coordinator, leave a message with your first and last name, zip code and the best number to reach you.

Are you eligible for Medicaid?

Our Certified Application Counselors are able to check your eligibility for Medicaid as well as assist with Marketplace Insurance. Ohio has granted permission for community health centers to participate in Presumptive Eligibility (PE) Program. The PE program allows us to provide immediate access to health services by giving our patients and their families, temporary health coverage through Medicaid if they are presumed to be eligible by us under the current Ohio Medicaid guidelines.

To apply for temporary and ongoing Medicaid you will need to bring: 1 month of household income (current within the last 30 days), social security numbers for all family members requesting assistance, and identification for all family members requesting assistance.
