If you are the parent or permanent legal guardian of a patient that is a child between the ages of 0-11 years old or a legal guardian of an adult patient or a person who has a Power of Attorney for healthcare over an adult patient, you can request proxy access.
Once a patient portal consent form is completed to request proxy access and proof of guardianship or Power of Attorney has been submitted to a Vision Front desk staff member, you will be provided a security token that lets you access the portal.
When your child turns 12, you will no longer be able to view any new information about your child in the portal. At this time, proxy access is not available for children between the ages of 12 to 17 years old.
1) Can I self-register for the portal online?
Unfortunately, we are unable to accept self-registrations online at this time. Please request a security token to access your vision patient portal account at one of our Vision locations.
2) What if I forget my password?
Go to https://www.nextmd.com, click “Need help with your username and password”, and follow the directions to reset your password.
3) What if I lost my token?
Please request a new security token at one of our Vision locations.
4) Do I need an email address to access my portal account?
No. You can use any computer or device to go to www.nextmd.com and follow the prompts to create your account with a security token and click “I do not have an email address”.
5) How do I access my account from my mobile device?
You can access your vision patient portal account on your iPhone/iPad or Android device by going to https://www.nextmd.com/m.
