Health and housing are inescapably linked. The social determinants of health (SDoH) include your housing, neighborhood and built environment, meaning that these factors directly contribute to a person’s health outcomes. Community health centers help patients overcome housing and other barriers to improve their health. This is why PrimaryOne Health is so proud to be a health center during this momentous week.
Our Healthcare for the Homeless (HCH) program has been consistently serving over 1,100 patients over the last three years. We partner with over 20 community-based organizations and agencies providing direct services to the individuals who are homeless. HCH engages individuals and families and expedites access to healthcare and support services. The Healthcare for Homeless team represent a voice for the voiceless, who are often suffering due to a lack of housing, economic hardship, mental health and/or addiction disorders. We must never forget that everyone we serve is a human being in need of assistance. When a patient who is homeless succeeds, PrimaryOne Health succeeds.
