For your convenience, we offer a multitude of ways to sign up for an appointment. Our patient portals are specifically designed to keep track of all appointments future. You will also be able to see notes regarding past appointments. While the portal is the preferred method of scheduling, phone and emergency options are also available.

What to Bring to Your First Appointment

We, at PrimaryOne Health, ask that you provide a valid photo ID, insurance card or proof of income, social security number, all current medications (including over the counter medication), name and address of previous Primary Care Provider to your first appointment. Proof of income can include any of the following: Pay stubs of one month of gross household earnings, unemployment benefits, workers compensation benefits, social securing benefits (with award letter), child support, alimony and tax documents (W2s, 1099, tax returns with adjusted gross income). How to Pay for Your Appointment

PrimaryOne aims to make healthcare both affordable and easy to access. For this reason, we accept a wide variety of payment options from our patients. These options include cash, credit card, debit card, check, the majority of health insurance plans by major providers and Medicaid. We also offer a sliding fee scale and onsite counseling to help determine Medicaid Eligibility. Sliding fee and insurance co-payments will be collected at the time of service.

Interpreting Services for Many Languages

As to further extend our services to more individuals and families in the Central Ohio area, we offer a variety of interpreting services. These methods include in-person translators, telephone conferencing and video conferencing to hurdle language barriers and help provide clarity.
